We do not collect Social Security numbers, and we recommend you avoid including one in any form or communication with us. If you believe you have previously provided a Social Security number to our practice, please contact us so we can securely delete it.

Wellness Client Information Is Not a Medical Record
Wellness services offered by Sacred Lotus Family Therapy, Inc. are non-clinical and do not create a treatment relationship. Information collected in connection with wellness services is not protected health information under HIPAA, but we treat it as confidential and will not disclose it to third parties except as described in this policy, as required by law, or with your consent. Data Security We implement reasonable technical and organizational measures to protect personal information from unauthorized access, use, or disclosure. No method of electronic transmission or storage is completely secure, and we cannot guarantee absolute security.
